Objetivo: evaluar el perfil de seguridad de nirmatrelvir-ritonavir (NMV-r) en la práctica clínica real y analizar la relevancia clínica de las interacciones farmacológicas en el desarrollo de eventos adversos. Material y métodos: estudio observacional, retrospectivo en el que se evaluaron los datos de seguridad de pacientes tratados con NMV-r entre abril y julio de 2022. Se recopilaron datos demográficos y analíticos antes de comenzar el tratamiento. La duración del seguimiento fue de 28 días y se evaluó el número reacciones adversas reportadas, así como si fueron manejadas de forma ambulatoria o precisaron de asistencia sanitaria especializada y la presencia de deterioro de la función renal y hepática. Se revisó el tratamiento concomitante, identificando interacciones farmacológicas teóricas (IFT) cuya gravedad fue definida mediante la clasificación Lexi-interact. Resultados: el estudio incluyó 146 pacientes, 82 (56,16 %) eran mujeres, cuya mediana de edad fue de 65 años (22-95). El número de IFT detectadas y mantenidas durante el tratamiento con NMV-r fue de 164, siendo el porcentaje de pacientes con al menos una interacción de 62,33%. La mediana de IFT por paciente fue de uno (0-5). En 18 pacientes (11,84%) se reportó al menos un evento adverso (EA). Once EA se relacionaron potencialmente con alguna IFT, 7 pacientes requirieron contacto con asistencia hospitalaria para el manejo del EA, 8 pacientes presentaron deterioro de la función renal y 2 de la función hepática a los 28 días. Los principales grupos de fármacos implicados en la aparición de algún EA fueron los anticoagulantes orales, así como los calcio-antagonistas. Conclusiones: nuestros resultados muestran un elevado número de IFT detectadas entre NMV-r y otros fármacos, aunque la frecuencia de EA asociados fue baja. Este estudio proporciona un mayor conocimiento de los fármacos implicados en dichas interacciones y su potencial relación con la aparición de EA.(AU)

The median number of TDDIs per patient was 1 (0-5). At least 1 adverse event (AE) was reported in 18 patients (11,84%). Eleven AEs were potentially related to any TDDI. Seven patients required contact with hospital assistance for AE management. Eight patients had impaired renal function and 2 had impaired liver function at 28 days. The main groups of drugs implicated in the occurrence of an AE were oral anticoagulants and calcium antagonists. Conclusions: Our results show a high number of TDDIs detected were detected between NMV-r and other drugs. This study provides greater knowledge of the drugs involved in such interactions and their potential relationship with the occurrence of adverse events.(AU)

Introducción: El Patient Health Questionnaire for Depression and Anxiety (PHQ-4) es una medida de autoinforme ultrabreve de síntomas depresivos y ansiosos. El PHQ-4 no ha sido adaptado ni validado en la población quechua hablante. Objetivo: Este estudio tuvo como objetivo adaptar y validar el PHQ-4 en una muestra de población peruana quechua hablante en su variante Collao de Puno-Perú. Metodología: Participaron 221 personas hablantes de quechua variante collao (promedio de edad de 31,2 años, SD = 11,7, 47,1 % hombres y 52,9 % mujeres). Se evaluó la evidencia de validez basada en la estructura interna, mediante el análisis factorial confirmatorio (CFA), la fiabilidad con el método de consistencia interna y las características de dificultad y discriminación de los ítems en base a la Teoría de Respuesta al Ítem (IRT). Resultados: Los resultados indicaron que todos los ítems del PHQ-4 son relevantes, coherentes y claros para representar adecuadamente los síntomas de ansiedad y depresión. El resultado del AFC apoyó un modelo unifactorial, donde los ítems de ansiedad y depresión conforman una sola variable latente con una fiabilidad alta. Todos los ítems del PHQ-4 presentas buenos parámetros de discriminación y los parámetros de dificultad indican que los ítems son comparativamente difíciles. Por lo tanto, se necesita una mayor presencia del rasgo latente (ansiedad y depresión) para responder las categorías de respuesta más altas. Discusión: En conclusión, la PHQ-4 presentó buenas propiedades psicométricas como una medida de detección primaria rápida, confiable y válida para personas quechuahablantes que necesitan una evaluación profunda, monitoreo de los síntomas para diagnóstico y tratamiento para la ansiedad o depresión.


Introduction: The Patient Health Questionnaire for Depression and Anxiety (PHQ-4) is an ultra-brief self-report measure of depression and anxiety symptoms. The PHQ-4 has not been adapted or validated in the Quechua-speaking population. Objective: This study aimed to adapt and validate the PHQ-4 in a sample of the Peruvian Quechua-speaking population with its Collao variant from Puno-Peru. Methods: A total of 221 Quechua speakers of the Collao variant (mean age 31.2 years, SD = 11.7, 47.1% male and 52.9% female) participated. Evidence of validity based on internal structure was evaluated using confirmatory factor analysis (CFA), reliability using the internal consistency method, and item difficulty and discrimination characteristics based on Item Response Theory (IRT). Results: The results indicated that all items of the PHQ-4 were relevant, consistent, and clear enough to adequately represent anxiety and depressive symptoms. The CFA results supported a one-factorial model, in which anxiety and depression items form a single latent variable with high reliability. All items of the PHQ-4 had good discrimination parameters, and the difficulty parameters indicated that the items were comparatively difficult. Therefore, a higher presence of the latent trait (anxiety and depression) is needed to answer higher response categories. Discussion: In conclusion, the PHQ-4 presented good psychometric properties as a rapid, reliable, and valid primary screening measure for Quechua speakers in need of in-depth assessment and symptom monitoring for the diagnosis and treatment of anxiety and depression.

BACKGROUND: Depression and anxiety are two of the most prevalent and disabling mental disorders worldwide, both in the general population and in outpatient clinical settings. OBJECTIVE: This study aimed to analyze the psychometric properties of the Patient Health Questionnaire-4 (PHQ-4) based on network analysis metrics. METHODS: A total of 911 Paraguayans (23.71% women and 76.29% men; mean age 31.25 years, SD = 10.63), selected by non-probabilistic convenience sampling, participated in the study. Network analysis was used to evaluate the internal structure, reliability, and measurement invariance between men and women. RESULTS: The results revealed that the PHQ-4 is a unidimensional measure through Exploratory Graph Analysis (EGA). Reliability, through structural consistency, identified that 100% of the time, only a single dimension was obtained, and all items remained stable, as they were always replicated within the empirical dimension. The unidimensional structure has shown evidence of configural invariance; therefore, the network structure functioned equally among the different sex groups. CONCLUSION: The PHQ-4 presented optimal preliminary evidence of validity based on its internal structure, reliability, and invariance between sexes. Therefore, it may be useful as an accurate and brief measure of anxiety and depressive symptoms in the Paraguayan context.

Introduction. The connection between news sources and the connotation of mental health articles in Paraguay during the COVID-19 pandemic (2020-2023) significantly impacted public opinion about mental health. Objective.This study analyzednews onmental health in Paraguay in the context of the COVID-19 pandemic. Material and methods.A descriptive retrospective study of all news articles on mental health published in the three primary newspapers of the country between May 2020 and May 2023 was conducted. The keywords were depressive disorders, anxiety disorders, bipolar disorder, schizophrenia, and mental health issues. Results. A total of 534 newswereexamined, 39.5% published by La Nación newspaper, 33.1% in 2023, and in 88.4% the author's identity and gender were not specified. Of the writing characteristics, 99.8% were written in the third person, 62.7% had a human-interest context,48.5% had a neutral connotation, 65.7% focused on health and well-being,and 7.5% written by health experts. The year of publication wasstatistically associated to connotation (p=0.003) and context (p<0.001). Conclusion. The media has paid significant attention to mental health. Despite the objectivity provided by third-person narration, problems arise because of a lack of author identification. To encourage balanced and non-polarized reporting, it is crucial to incorporate expert voices and promote the framing of mental health stories as "human interest". Introducción. La conexión entre las fuentes de noticias y la connotación de los artículos sobre salud mental en Paraguay durante la pandemia de COVID-19 (2020-2023) impactó significativamente en la opinión pública sobre la salud mental. Objetivo. Analizar las noticias sobre salud mental en Paraguay en el contexto de la pandemia COVID-19. Material y métodos. Se realizó un estudio descriptivo retrospectivo de todos los artículos periodísticos sobre salud mental publicados en los tres principales periódicos del país entre mayo de 2020 y mayo de 2023. Las palabras clave fueron trastornos depresivos, trastornos de ansiedad, trastorno bipolar, esquizofrenia y problemas de salud mental. Resultados. Se examinaron 534 artículos periodísticos, 39,5% publicados por el diario La Nación, 33,1% en 2023y en 88,4%no se especificó la identidad ni el género del autor. De las características de redacción, el 99,8% estaban escritos en tercera persona, 62,7% pertenecían a un contexto de interés humano,48,5% tenían una connotación neutra, 65,7% se centraban en la salud y el bienestar y el 7,5% escritos por expertos en salud. El año de publicación se asoció significativamente con la connotación (p=0,003) y con el contexto (p<0,001). Conclusión. Los medios de comunicación han prestado gran atención a la salud mental. A pesar de la objetividad que aporta la narración en tercera persona, surgen problemas por la falta de identificación del autor. Para fomentar una información equilibrada y no polarizada, es crucial incorporar voces expertas y promover el encuadre de las historias de salud mental como "interés humano". BACKGROUND: Pandemic fatigue generates low motivation or the ability to comply with protective behaviors to mitigate the spread of COVID-19. AIMS: This study aimed to analyze the symptoms of pandemic fatigue through network analysis in individuals from five South American countries. METHOD: A total of 1,444 individuals from Argentina, Bolivia, Paraguay, Peru, and Uruguay participated and were evaluated using the Pandemic Fatigue Scale. The networks were estimated using the ggmModSelect estimation method and a polychoric correlation matrix was used. Stability assessment of the five networks was performed using the nonparametric resampling method based on the case bootstrap type. For the estimation of network centrality, a metric based on node strength was used, whereas network comparison was performed using a permutation-based approach. RESULTS: The results showed that the relationships between pandemic fatigue symptoms were strongest in the demotivation dimension. Variability in the centrality of pandemic fatigue symptoms was observed among participating countries. Finally, symptom networks were invariant and almost identical across participating countries. CONCLUSIONS: This study is the first to provide information on how pandemic fatigue symptoms were related during the COVID-19 pandemic.

El tema de la salud mental en niños y adolescentes requiere mayor atención. Por lo tanto, este estudio tuvo como objetivo determinar las características de los pacientes de consulta externa de un centro especializado de un hospital universitario paraguayo. Este estudio empleó una serie retrospectiva, no probabilística, de casos consecutivos. Se examinaron las historias clínicas de pacientes de ambos sexos, de entre 2 y 17 años de edad, que solicitaron consulta en el Departamento de Psiquiatría del Niño y del Adolescente del Hospital de Clínicas entre julio de 2021 y julio de 2023, y que fueron diagnosticados con un trastorno mental según la CIE-10. Se recogieron datos demográficos y clínicos de 447 pacientes, representando 1278 consultas. La mayoría de los pacientes eran mujeres (57,7%), con una edad media de 13 ± 3 años. La mayoría de los pacientes (57,3%) procedían del Departamento Central, el 61,5% eran de zonas rurales y el 72% no tenían seguro médico. Los diagnósticos más frecuentes fueron episodios depresivos (F32), otros trastornos de ansiedad (F41), trastornos de conducta (F91) y trastornos específicos de la personalidad (F60). Los resultados de este estudio proporcionan información crucial sobre la salud mental de niños y adolescentes, que es esencial para informar las políticas de salud mental, asignar recursos y formar a los profesionales sanitarios encargados de atender a este grupo de pacientes.


The topic of mental health in children and adolescents requires increased attention. Therefore, this study aimed to determine the characteristics of outpatient clinic patients at a specialized center within a Paraguayan University Hospital. This study employed a retrospective, non-probabilistic series of consecutive cases. The medical records of patients of both sexes, aged between 2 and 17 years, who sought consultation at the Department of Child and Adolescent Psychiatry of the Hospital de Clínicas between July 2021 and July 2023, and were diagnosed with a mental disorder according to the ICD-10, were examined. Demographic and clinical data were collected from 447 patients, representing 1278 consultations. The majority of the patients were female (57.7%), with a mean age of 13 ± 3 years. Most patients (57.3%) were from the Central Department, 61.5% were from rural areas, and 72% did not have medical insurance. The most frequent diagnoses were depressive episodes (F32), other anxiety disorders (F41), conduct disorders (F91), and specific personality disorders (F60). The findings of this study provide crucial information on child and adolescent mental health, which is essential for informing mental health policies, allocating resources, and training healthcare professionals tasked with caring for this patient group.

BACKGROUND: Positive mental health includes not only the absence of mental disorders but also the presence of subjective well-being, good coping strategies for life stress, and strategies for adapting to community life. It is well known that the COVID-19 pandemic has challenged mental health in general population worldwide. However, research has not measured protective factors for mental health in the general population after the declared end of pandemic by the World Health Organization. METHODS: This observational, cross-sectional study surveyed 591 Paraguayan participants aged ⩾18 years, who were recruited through an online survey. Demographic characteristics, socioeconomic status were collected as well as mental health and validated tools for hope, resilience, subjective happiness were administered. RESULTS: Of the participants, 81.6% were women, 54% were married or in a relationship and 90.7% reported an university education. The main source of stress was economic issues (30.3%). A total of 22.7% had been previously diagnosed with a mental disorder, 22.2% had consulted a mental health professional and 10.8% had consumed prescription drugs. 42.6% reported flourishing, 36.2% reported moderate and 21.2% reported languishing mental health. CONCLUSIONS: This large survey has shown that most of participants reported a flourishing mental health with high ratings at hope, resilience, and subjective happiness scales. Also, the main sources of stress were economic issues, not consequently related to the pandemic. This may add evidences to the international debate on the long term effects of the global pandemic and probably suggests that recovery processes have been collectively adopted in Paraguay.

Introducción: la gestión eficiente del tiempo se erige como una competencia cardinal entre los estudiantes de Medicina. Esta destreza resulta imperativa, dado que les habilita para administrar sus agendas de forma eficaz, evitando así el estrés inherente a su formación y asegurando un desempeño exitoso, mejorando así el equilibrio entre sus compromisos académicos y personales al adoptar decisiones pertinentes, tales como la priorización efectiva de tareas y la prevención de situaciones de sobrecarga que puedan menoscabar su rendimiento. Metodología: estudio observacional descriptivo, de prevalencia y asociación cruzada, temporalmente prospectivo, realizado en estudiantes de Medicina de Paraguay, con el fin de determinar el manejo de su tiempo. Para medir el manejo del tiempo se usó la versión en español de la Time Management Behavior Questionnaire for University Students que es un instrumento autoadministrado que integra 34 ítems relacionados con la forma en que los estudiantes manejan su tiempo de aprendizaje y estudio. Resultados: la correlación entre las variables cuantitativas demostró relación entre edad y la subescala "establecimiento de objetivos y prioridades" (r= -0,208; p=0,038), siendo esta asociación indirecta y débil. En cuanto a diferencia de medias, se encontró relación entre sexo y la subescala "herramientas de gestión del tiempo" (t=-2,37; gl=98; p=0,020), observándose que los hombres (2,61 ± 0,89) tenían una media menor que las mujeres (3,04 ± 0,74). Conclusiones: el aspecto que obtuvo la puntuación más baja fue la preferencia hacia la desorganización. Esta característica, contrastada con la alta puntuación otorgada al establecimiento de metas y prioridades, señala una tendencia hacia un enfoque de aprendizaje estructurado y ordenado, procurando así evitar la carencia de planificación en las actividades.


Introduction: efficient time management is emerging as a core competency for medical students. Acquiring this skill enables them to effectively manage their agendas, thus avoiding the inherent stress in their training and ensuring successful performance. It also improves their ability to balance their academic and personal commitments by improving their decision-making skills, such as effective task prioritization and preventing task-overload situations that may undermine their performance. Methodology: this was a descriptive, observational study of prevalence and cross-association conducted in medical students in Paraguay to assess their time management skills. To measure time management, the Spanish version of the Time Management Behavior Questionnaire for University Students was used. This is a self-administered instrument that includes 34 items related to how students manage their learning and study time. Results: the correlation between quantitative variables showed a relationship between age and the "setting goals and priorities" subscale; (r= -0.208; p=0.038); this association was indirect and weak. In terms of mean differences, a relationship was found between sex and the "time management tools" subscale; (t=-2.37; gl=98; p=0.020), with men (2.61 ± 0.89) having a lower mean than women (3.04 ± 0.74). Conclusions: the aspect that scored the lowest was the tendency towards disorganization. This characteristic, in contrast to the high score seen in goal setting and prioritization, shows a tendency towards a structured and orderly approach to learning, thus avoiding a lack of planning in students' activities.

Introducción: el cronotipo es la predisposición natural de cada individuo a experimentar picos de energía o momentos de descanso en función de su ritmo circadiano. Los individuos pueden tener cronotipos matutinos, vespertinos o intermedios, que pueden tener relación con el desarrollo de depresión. Objetivo: determinar la existencia de asociación entre el cronotipo y la sintomatología depresiva en una muestra de adultos. Metodología: este estudio observacional, descriptivo y exploratorio se realizó en agosto de 2023. Se incluyeron en este estudio adultos de ambos sexos. Se recolectaron datos sociodemográficos, y los participantes respondieron el Morningness-Eveningness Questionnaire de Horne y Östberg para evaluar el cronotipo, y el Patient Health Questionnaire de 2 ítems (PHQ-2) para evaluar los síntomas depresivos. Se utilizaron estadísticas descriptivas para todas las variables. Para la estadística inferencial, se utilizó la prueba de chi-cuadrado con un nivel de confianza del 95 %. Se halló una OR=2,5 (IC 95%: 1,05-5,95) a favor del cronotipo vespertino para el desarrollo de depresión. Conclusión: casi siete de cada diez participantes tenían un cronotipo matutino, mientras que tres de cada diez dieron positivo en depresión. Los individuos con cronotipos vespertinos son más propensos a desarrollar síntomas depresivos que aquellos con cronotipos matutinos.


Introduction: chronotype is the natural predisposition of each individual to experience peaks of energy or moments of rest according to their circadian rhythm. Individuals may have morning, evening, or intermediate chronotypes that may be related to the development of depression. Objective: to determine the existence of an association between chronotype and depressive symptomatology in a sample of adults. Methodology: this observational, descriptive, and exploratory study was conducted in August 2023. Adults of both sexes were included in this study. Sociodemographic data were collected, and participants answered the Morningness-Eveningness Questionnaire by Horne and Östberg to assess chronotype, and the 2-item Patient Health Questionnaire (PHQ-2) to assess depressive symptoms. Descriptive statistics were used for all the variables. For inferential statistics, the chi-squared test was used with a confidence level of 95 %. Odds ratio was used to quantify the relationships. Results: a total of 105 people participated in the study, 58 % of whom were women. The mean age of the sample was 29.4 ± 11.6 years. Sixty-nine percent of the sample presented the morning chronotype, while 31 % presented the evening chronotype. It was found that 31 % of the participants scored positively for depression (PHQ-2 ≥ 3). A relationship was found between chronotype and PHQ-2 cutoff points (p=0.036). An OR=2.5 (95 % CI 1.05-5.95) was found in favor of the evening chronotype for the development of depression. Conclusion: almost seven out of ten participants had a morning chronotype, whereas three out of ten were positive for depression. Individuals with evening chronotypes are more likely to develop depressive symptoms than those with morning chronotypes.

Introducción: la salud mental de los estudiantes de medicina ha sido ampliamente investigada, demostrando que forman parte de un grupo vulnerable al desarrollo de trastornos mentales. Objetivo: el objetivo de esta investigación fue determinar la frecuencia de NSSI y sus factores asociados en estudiantes de medicina del Paraguay. Metodología: este fue un estudio descriptivo y transversal. Se lanzó una encuesta online para evaluar depresión, ansiedad y autolesiones, se utilizaron las escalas PHQ-2, GAD-7 y SHQ, respectivamente. Resultados: recibimos respuestas de 330 estudiantes de medicina. De los participantes, el 71,2 % eran mujeres. Se identificó que el 46,4 % de los participantes padecía depresión (PHQ-2 ≥ 3) y el 37,3 % ansiedad (GAD-7 ≥10). La frecuencia de NSSI fue del 27 % (n = 89). Los principales factores asociados a NSSI fueron un diagnóstico previo de un trastorno mental (que aumentó la probabilidad de NSSI en 3,76 veces) y/o una historia de abuso físico o sexual (con un aumento de 3,75 veces). Conclusión: esta investigación encontró la presencia de NSSI en casi 3 de cada 10 de los estudiantes de medicina encuestados. Los principales factores asociados con la conducta autolesiva fueron un diagnóstico previo de un trastorno mental y/o antecedentes de abuso físico o sexual.

Mientras que la investigación primaria implica la participación activa del propio investigador, la investigación secundaria consiste en la síntesis y el análisis de los resultados organizados y publicados por otros como investigaciones primarias. En Medicina, una investigación primaria busca llegar a una conclusión válida para mejorar la salud humana, utilizando el método científico. Por lo tanto, se requiere de una investigación bien diseñada, que se sostenga sobre la base sólida de una metodología detallada y regida por principios clínicos éticos. Esto es importante, puesto que en las Ciencias Médicas solo pueden extraerse conclusiones significativas a partir de los datos recogidos y los resultados obtenidos a través de un diseño científico adecuado. Por lo tanto, la selección de un diseño de investigación correcto es importante para responder, de manera imparcial y científica, a las preguntas que el investigador se ha planteado. En este artículo de revisión se presenta una propuesta de algoritmo de clasificación de los diseños de investigación primaria. Asimismo, se discuten los distintos tipos de diseño utilizados en investigación primaria médica, desde el paradigma cuantitativo. Se ilustran, con ejemplos, las características clave que hacen al concepto, aspectos metodológicos y ventajas y limitaciones, así como las posibilidades de uso de los distintos tipos de diseño. Conocer todo esto es crucial, puesto que la calidad, fiabilidad y posibilidad de publicar una investigación en Medicina dependen decisivamente de la selección de un diseño de investigación adecuado.


While primary research involves the active participation of the researcher, secondary research consists of the synthesis and analysis of results organized and published by others as primary research. In medicine, primary research has sought to reach a valid conclusion to improve human health using scientific methods. Therefore, primary research requires well-designed research that is sustained on a sound basis of detailed methodology and governed by ethical clinical principles. This is important, since in the Medical Sciences, meaningful conclusions can only be drawn from the data collected and the results obtained through an appropriate scientific design. Therefore, the selection of a correct research design is important to answer the questions posed by the researcher in an unbiased and scientific manner. In this review article, an algorithm for classifying primary research designs is presented. It also discusses the different types of designs used in primary medical research based on the quantitative paradigm. The key features of the concept, methodological aspects, advantages, and limitations, as well as the possibilities of using different designs, are illustrated with examples. This knowledge is crucial since the quality, reliability, and publishability of research in medicine depend decisively on the selection of an appropriate research design.

Introducción: la gestión eficiente del tiempo se erige como una competencia cardinal entre los estudiantes de Medicina. Esta destreza resulta imperativa, dado que les habilita para administrar sus agendas de forma eficaz, evitando así el estrés inherente a su formación y asegurando un desempeño exitoso, mejorando así el equilibrio entre sus compromisos académicos y personales al adoptar decisiones pertinentes, tales como la priorización efectiva de tareas y la prevención de situaciones de sobrecarga que puedan menoscabar su rendimiento. Metodología: estudio observacional descriptivo, de prevalencia y asociación cruzada, temporalmente prospectivo, realizado en estudiantes de Medicina de Paraguay, con el fin de determinar el manejo de su tiempo. Para medir el manejo del tiempo se usó la versión en español de la Time Management Behavior Questionnaire for University Students que es un instrumento autoadministrado que integra 34 ítems relacionados con la forma en que los estudiantes manejan su tiempo de aprendizaje y estudio. Resultados: la correlación entre las variables cuantitativas demostró relación entre edad y la subescala "establecimiento de objetivos y prioridades" (r= -0,208; p=0,038), siendo esta asociación indirecta y débil. En cuanto a diferencia de medias, se encontró relación entre sexo y la subescala "herramientas de gestión del tiempo" (t=-2,37; gl=98; p=0,020), observándose que los hombres (2,61 ± 0,89) tenían una media menor que las mujeres (3,04 ± 0,74). Conclusiones: el aspecto que obtuvo la puntuación más baja fue la preferencia hacia la desorganización. Esta característica, contrastada con la alta puntuación otorgada al establecimiento de metas y prioridades, señala una tendencia hacia un enfoque de aprendizaje estructurado y ordenado, procurando así evitar la carencia de planificación en las actividades.


Introduction: efficient time management is emerging as a core competency for medical students. Acquiring this skill enables them to effectively manage their agendas, thus avoiding the inherent stress in their training and ensuring successful performance. It also improves their ability to balance their academic and personal commitments by improving their decision-making skills, such as effective task prioritization and preventing task-overload situations that may undermine their performance. Methodology: this was a descriptive, observational study of prevalence and cross-association conducted in medical students in Paraguay to assess their time management skills. To measure time management, the Spanish version of the Time Management Behavior Questionnaire for University Students was used. This is a self-administered instrument that includes 34 items related to how students manage their learning and study time. Results: the correlation between quantitative variables showed a relationship between age and the "setting goals and priorities" subscale; (r= -0.208; p=0.038); this association was indirect and weak. In terms of mean differences, a relationship was found between sex and the "time management tools" subscale; (t=-2.37; gl=98; p=0.020), with men (2.61 ± 0.89) having a lower mean than women (3.04 ± 0.74). Conclusions: the aspect that scored the lowest was the tendency towards disorganization. This characteristic, in contrast to the high score seen in goal setting and prioritization, shows a tendency towards a structured and orderly approach to learning, thus avoiding a lack of planning in students' activities.

Se recolectaron datos sociodemográficos, y los participantes respondieron el Morningness-Eveningness Questionnaire de Horne y Östberg para evaluar el cronotipo, y el Patient Health Questionnaire de 2 ítems (PHQ-2) para evaluar los síntomas depresivos. Se utilizaron estadísticas descriptivas para todas las variables. Para la estadística inferencial, se utilizó la prueba de chi-cuadrado con un nivel de confianza del 95 %. OBJECTIVE: The objective of the study is to evaluate the characteristics and outcomes of patients with abdominal aortic aneurysm (AAA) and its correlation with mortality in the first 30 days after the procedure was performed. METHODS: Demographic information, clinical and radiological characteristics, as well as outcomes 30 days after the procedure was performed were assessed and compared. Continuous variables were analyzed with Student's t-test and categorical with Chi-square and Fisher's exact test. RESULTS: Duration of the procedure (p = 0.001), blood loss (p < 0.001), age > 75 years (p = 0.027), aneurysm size > 65 mm (p = 0.01), open surgery (p = 0.001), presence of pain (p = 0.005), chronic kidney disease (p = 0.03), and rupture of the aneurysm (p < 0.001) were the factors significantly associated with mortality. CONCLUSION: It is essential that patient characteristics and comorbidities are assessed, as well as factors that may affect the outcomes to predict the prognosis in patients with AAA. At present, no mortality predictive model is universally applicable and highly variable performance across different populations might need a model that adapts to the population of interest.


OBJETIVO: Evaluar las características y resultados de los pacientes con aneurisma de aorta abdominal y su correlación con la mortalidad en los primeros 30 días después de realizado el procedimiento. MÉTODOS: Se evaluó y comparó la información demográfica, las características clínicas y radiológicas, así como los resultados a los 30 días de realizado el procedimiento. Las variables continuas se analizan con la prueba de t de Student y las categóricas con Chi-cuadrado y la prueba exacta de Fisher. RESULTADOS: La duración del procedimiento (p = 0.001), pérdida de sangre (p < 0.001), edad > 75 años (p = 0.027), tamaño del aneurisma > 65 mm (p = 0.01), cirugía abierta (p = 0.001), presencia de dolor (p = 0.005), enfermedad renal crónica (p = 0.03) y rotura del aneurisma (p < 0.001) fueron los factores asociados significativamente a la mortalidad. CONCLUSIÓN: Es fundamental evaluar las características de los pacientes y las comorbilidades, así como los factores que pueden afectar los resultados para predecir el pronóstico en pacientes con aneurisma de aorta abdominal. En la actualidad, ningún modelo predictivo de mortalidad es universalmente aplicable y la alta variabilidad de resultados entre diferentes poblaciones podría necesitar un modelo que se adapte a la población de interés.

Introducción: La investigación sobre la autolesión no suicida permite aumentar la conciencia sobre este fenómeno, posibilita su detección temprana y la implementación de estrategias más eficaces en prevención y tratamiento. Esto conlleva a una reducción del sufrimiento individual, los costos económicos y el impacto en la sociedad en su totalidad. Objetivo: Determinar la frecuencia y factores asociados a autolesión no suicida en una muestra de población adulta de Paraguay. Metodología: Estudio descriptivo de asociación cruzada, de corte transversal y prospectivo. Una encuesta en línea fue difundida a través de redes sociales y aplicaciones de mensajería. Esta incluía preguntas sobre datos sociodemográficos y clínicos. La presencia de autolesiones se determinó a través de la versión en español de la escala Self-Harm Questionnaire. Se utilizó estadística descriptiva para todas las variables. Para buscar asociaciones se utilizó la prueba chi cuadrado. Resultados: Participaron 241 personas (media de edad=32±12 años, 74,7% mujeres). Se encontró una frecuencia de autolesiones de 24,5% (n=59). Introduction: Research on non-suicidal self-injury raises awareness of this phenomenon, enabling the early detection and implementation of more effective strategies in prevention and treatment. This leads to a reduction in individual suffering, economic costs, and impact on society. Objective: To determine the frequency and associated factors of non-suicidal self-injury in a sample of the adult population of Paraguay. Methodology: This was a descriptive, cross-sectional, and prospective study. An online survey was disseminated through social networks and messaging applications. The survey included questions on sociodemographic and clinical data. The presence of self-injury was determined by using the Spanish version of the Self-Harm Questionnaire. OBJECTIVE: The aim of the study was to evaluate the safety profile of nirmatrelvir-ritonavir (NMV-r) in real clinical practice and to analyse the clinical relevance of drug-drug interactions in the development of adverse events. METHODS: Observational, retrospective study in which safety data of patients treated with NMV-r between April and July 2022 in an outpatient setting were evaluated. The duration of follow-up was 28 days and the number of adverse reactions reported, as well as whether they were managed on an outpatient basis or required health care, and the presence of renal and hepatic function impairment were assessed. Concomitant treatment was reviewed, identifying theoretical drug-drug interactions (TDDIs) whose severity was defined using the Lexi-interact classification. RESULTS: The study included 146 patients. 82 (56.16%) were women, whose median age was 65 years (22-95). the number of TDDIs detected and maintained during treatment with NMV-r was 164, with the percentage of patients with at least 1 interaction being 62.33%. The median number of TDDIs per patient was 1 (0-5). At least 1 adverse event (AE) was reported in 18 patients (11.84%). 11 AEs were potentially related to any TDDI. 7 patients required contact with hospital assistance for AE management. 8 patients had impaired renal function and 2 had impaired liver function at 28 days. The main groups of drugs implicated in the occurrence of an AE were oral anticoagulants and calcium antagonists. CONCLUSIONS: Our results show a high number of TDDIs detected were detected between NMV-r and other drugs. This study provides greater knowledge of the drugs involved in such interactions and their potential relationship with the occurrence of adverse events.

Dermatitis artefacta (DA) is a psycho-dermatologic condition based on patients' behavioral patterns, characterized by an intentional production of cutaneous lesions on their own skin. The clinical presentation can be highly variable. Patients with DA seldom seek psychological support or psychiatric consultation. More often, they seek help from their primary care physician or dermatologist. This review article aims to provide a practical guide for the diagnosis and management of AD and affected patients. A broad literature search was performed using the PubMed and Google Scholar electronic online databases, using key words "dermatitis artefacta", "diagnosis", "management", and "psychodermatology". The search was limited to English and Spanish language articles and was supplemented with themed books and book chapters. DA can occur in a variety of clinical presentations, and physicians should suspect DA in patients with a history of psychiatric disorders or extensive use of healthcare services. The ultimate goal of DA treatment may be a proper referral to mental health services. However, the prognosis is poor even when successful mental health referrals are achieved, with low recovery rates. A useful approach may include the suggestion that a mental health provider can help with the anxiety and the distress generated by the lesions: in this case in this case it will be crucial to discuss this with the mental health provider after obtaining informed consent from the patient. Considering the difficulty in promoting patients' adherence to treatment, the ideal setting for DA treatment is a psycho-dermatologic clinic, where both dermatologic and psychological interventions can be seamlessly integrated.

OBJECTIVES: Linezolid is an oxazolidin commonly related to the development of haematological toxicity, being renal clearance the major factor involved in the drug clearance. The aim of this study is to evaluate the influence of increased filtration rates in the incidence of linezolid-induced haematological toxicity by comparing augmented renal clearance (ARC) patients versus normal renal function patients. MATERIAL AND METHODS: A retrospective, observational study was conducted on hospitalized patients treated with linezolid for 5 days or more during 2014-2019 period. Patients with a filtration rate of ≥130 mL/min versus reference patients (60-90 mL/min) were compared. Haematological toxicity was defined as a decrease of 25% in platelets, of 25% in haemoglobin, and/or 50% in neutrophils from baseline. Toxicity relevance was classified according to Common Terminology Criteria for Adverse Events v5. Incidence of haematological toxicity between groups was studied by chi-square and Fisher test. Furthermore, percentage diminution of all 3 parameters was calculated and compared by Mann-Whitney test and treatment interruption and transfusion requirements were registered. RESULTS: 30 ARC patients and 38 reference patients were included. Haematological toxicity was observed in 16.66% of ARC patients vs 44.74% of reference patients (P=.014); thrombocytopenia in 13.33% vs 36.84% (P=.051), anaemia in 3.3% vs 10.52% (P=.374) and neutropenia in 10% vs 23.68% (P=.204). Median percentage of platelets decrease in ARC patients was -10.36 (-193.33-62.03) vs 2.68 (-163.16-82.71) in reference patients (P=.333), while haemoglobin decrease was 2.50 (-12.12-25.93) vs 9.09 (-17.72-30.63) (P=.047) and neutrophils decrease was 9.14 (-73.91-76.47) vs 27.33 (-86.66-90.90) (P=.093). 10.5% of normal renal function patients reported at least 1 adverse event grade 3 or superior while 2.6% of them interrupted treatment and 5.2% had transfusion requirements. No major events or interruptions were reported in ARC patients. CONCLUSION: Our findings suggest a lower incidence and clinical relevance of haematological toxicity in augmented renal clearance patients. Thrombocytopenia was the major event in both populations. This might be related to a lower exposure to the drug due to the higher clearance and likely lower therapeutic efficiency. These results suggest a potential benefit of therapeutic drug monitoring on high risk patients.
